Unholy Death Knight Guide
Unholy got a major Midnight rework that reshaped its flow around a new Lesser Ghoul / Putrefy engine layered on top of the classic disease + minion playstyle. The result is one of the strongest specs in the season: top-tier sustained AoE and pet-cleave damage make it an S-tier Mythic+ pick, while powerful single-target burst windows keep raid output high (A-tier). The skill ceiling lives in managing Lesser Ghoul stacks, plague uptime, and lining up Army of the Dead with Dark Transformation.
As of Patch 12.0.5, the Nebulous Voidcore lets you target gear via Bonus Loot rolls after Season 1 Raid bosses, Mythic+ dungeons, Bountiful Delves, and Nightmare Prey Hunts โ a controlled way to chase Unholy BiS instead of praying to normal drops. A Bonus Roll after a M+ dungeon awards the Great Vault item level for that keystone level, so +10 and above can give 1/6 Myth-track pieces. Save your weekly cores for the highest difficulty you can clear and spend them on the slots that move the needle most: trinkets and your tier set.

Apply diseases, build Festering Wounds / Lesser Ghoul stacks, then dump every cooldown together so Army of the Dead, Dark Transformation and your trinket all overlap. Pre-pull Raise Dead if your ghoul isn't already out.
- Outbreak to apply Virulent Plague / Dread Plague
- Festering Strike x2 to bank Festering Wound / Lesser Ghoul stacks
- Army of the Dead + Dark Transformation + trinket + racial + potion (stack the whole burst)
- Apocalypse to burst the wounds and summon transient ghouls (when talented)
- Putrefy x2 (consumes Lesser Ghouls โ highest priority inside Dark Transformation)
- Soul Reaper / Unholy Assault on cooldown inside the window
- Scourge Strike to spend wounds, Death Coil on Sudden Doom procs
- Putrefy x2 again, then fall into the single-target priority below

Diseases up at all times, Festering Wounds banked for burst, and Runic Power spent before it overcaps. Front-load Putrefy and wound spending into Dark Transformation windows.
- Keep Outbreak / diseases active (refresh before they fall off)
- Re-apply the Festering Scythe buff if it has dropped
- Soul Reaper on targets below 35% (or with a Reaping proc during Dark Transformation)
- Putrefy during Dark Transformation โ consumes Lesser Ghoul stacks for big damage
- Apocalypse / Unholy Assault on cooldown to detonate wounds
- Death Coil on Sudden Doom procs, or to avoid Runic Power overcap
- Festering Strike to maintain 2โ4 banked wounds / Lesser Ghoul stacks
- Scourge Strike to spend wounds (free Rune one after the 4pc proc)
- Death Coil as the Runic Power filler
๐ฅ AoE / Multi-Target Priority
Same engine as single-target, but stand in Death and Decay, spread diseases, and swap Death Coil for Epidemic as your Runic Power dump. Epidemic scales quadratically with the number of diseased targets โ keep plagues on everything.
- Death and Decay down and stay inside it (mandatory for AoE cleave)
- Outbreak / spread diseases to the whole pack (Epidemic feeds off these)
- Re-apply Festering Scythe if the buff expired
- Putrefy during Dark Transformation for the biggest cleave hit
- Apocalypse / Unholy Assault on cooldown
- Epidemic at high Runic Power or on Sudden Doom procs (your AoE spender)
- Soul Reaper on low-health priority targets during cooldowns
- Festering Strike / Festering Scythe to build and maintain wounds (4โ6 stacks)
- Scourge Strike (cleaves inside Death and Decay) to spend wounds
โฑ๏ธ Cooldowns
Everything stacks. Army of the Dead is your defining burst window โ line Dark Transformation up shortly after it so Commander of the Dead covers the full 30s, and dump Apocalypse, Soul Reaper and your trinket into the same window.
๐ Defensive Toolkit
Death Knight has one of the deepest defensive kits in the game โ anti-magic, a self-heal, a stun-immunity wall, and a panic Undead form. Anti-Magic Shell pre-cast can outright eat magic mechanics, so know your encounter timings.

๐ Stat Priority
Priority (both hero trees): Item Level / Strength > Mastery โ Critical Strike > Haste > Versatility. Raise Strength as high as possible through item level first, then balance Mastery and Crit โ keeping them roughly even is the safe default while you sim your own gear.
- Strength โ your primary; almost always favor higher item level over a secondary-stat reforge.
- Mastery โ boosts disease and shadow/minion damage; slightly more valuable for San'layn because of its minion interactions.
- Critical Strike โ strong and consistent, pairs evenly with Mastery.
- Haste โ smooths Rune regen and the rotation but sits below Mastery/Crit.
- Versatility โ lowest value (also adds a little DR), the dump stat for PvE.
๐ Tier Set, Trinkets & Consumables
Tier set: 2pc = Dread Plague and Virulent Plague damage increased by ~25%. 4pc = Putrefy deals ~20% increased damage and causes your next Scourge Strike to cost no Rune. Both bonuses are strong โ complete the 4pc before optimizing off-set secondary slots.
Top trinkets: Gaze of the Alnseer is far and away the best (unrivaled Strength). Light Company Guidon is the best on-use, syncing with your ~1.5 min burst. Heart of Ancient Hunger and the Radiant/Umbral Plume passives round out the options; Mythic+ alternatives include dungeon trinkets like Vessel of Tortured Souls.
Consumables: Flask of the Shattered Sun (or Flask of the Magisters by stat spread) ยท Potion of Recklessness combat / Light's Potential alt ยท Silvermoon Health Potion ยท Feast (Harandar Celebration / Silvermoon Parade) or Impossibly Royal Roast solo ยท Void-Touched Augment Rune ยท Thalassian Phoenix Oil on weapon.
Runeforge: Rune of the Apocalypse โ best in all content (synergizes with every Lesser Ghoul); Rune of the Fallen Crusader a hair behind. Gems: Indecipherable Eversong Diamond + Flawless Masterful Garnet / Flawless Deadly Amethyst (fill your lowest stat). Enchants: ring Eyes of the Eagle, chest Mark of the Worldsoul, legs Forest Hunter's Armor Kit, boots Lynx's Dexterity.
๐๏ธ Mythic+ Tips
- Stay inside Death and Decay on every pull โ your AoE wound-cleave and Epidemic only work from inside it.
- Spread diseases to the whole pack before dumping Epidemic; quadratic scaling means full plague coverage is a huge damage gain.
- Gorefiend's Grasp is your signature M+ button โ clump scattered packs for the group's AoE and your own cleave.
- Bank Festering Wounds before Apocalypse so it detonates a full payload; don't burn them on cooldown right before pressing it.
- Stagger Army of the Dead across pulls so a burst window lines up with each big trash group; line Dark Transformation just after it.
- San'layn's party Leech / Vampiric Speed is great for chained dangerous pulls; Rider of the Apocalypse is the simpler, consistent default.
- Use Anti-Magic Shell proactively to eat scripted magic casts, and Death Grip / Asphyxiate as backup interrupts behind Mind Freeze.

๐ซ Common Mistakes
- Letting diseases fall off โ every Epidemic, Putrefy and disease bonus collapses without full plague uptime.
- Pressing Apocalypse with no banked Festering Wounds โ it detonates wounds, so build them first.
- Misaligning Dark Transformation โ popping it too early so Commander of the Dead doesn't cover the full Army window.
- Overcapping Runic Power โ sitting at 100 RP is wasted Death Coil/Epidemic damage; spend before you cap.
- Standing outside your own Death and Decay in AoE โ you lose the wound-cleave and most of your multi-target damage.
- Saving cooldowns 'for later' โ Army, Dark Transformation and Apocalypse come up often; hold only briefly to line up an upcoming burst pull.
